Our Allied Health clinicians work within the community to help older persons stay safe and well at home for longer.
Allied Health at Home - HCP
Our allied health clinicians travel to their clients to provide therapy in their home or other location in the community, such as the pool, gym or park.
All members of our team are closely supported by senior clinicians with aged care experience, collaborate closely with one another, and engage in regular professional development to expand their professional knowledge and skills to continue to meet the unique needs of each of their clients.
We work closely with Home Care Package Providers and Case Managers, providing regular updates and progress reports to ensure that everyone in a client’s care team is on the same page.

How we work with our clients & their HCP providers

When we receive a referral, we respond on the same day to schedule a discussion with the client, or their HCP provider.

We call our new client or their HCP provider within the next business day to understand what is needed, so we can allocate the right practitioner.

At the intial 1-2 appointments, the allied health practitioner will carry out an assessment to understand our client’s goals, and exactly how we can support them.

Once the allied health therapist has established an understanding of the client’s needs, they will create and share a brief therapy plan outlining the goals of therapy, and how we will achieve them.

Our practitioners will continue to support our clients to work towards, and adjust where necessary, their health and wellbeing goals.
